EARLY CATARACT SURGERY

"CLEAR LENS EXTRACTION"

High-refraction disorders, who needs reading glasses (usually from 40 years), can be helped with a cataract surgery if they do not want to wear glasses anymore.
Even if they have no lens opacity, lens removal may still be performed.
After removing the lens, a custom lens can be apllied.
By normal individuals is that a lens with a strenght of 21diopters.
In persons with high myopia of -15, you can expect it around the value of + 6 (= 21 -15).
Because there are other parameters involved, the exact strength of the lens is calculated using a computer program, so we come very close to 0!
Perfection can not be promised. In any case, we can guarantee that the glasses, needed for far is after surgery less important than before: in 85% of the cases is the glasses that are needed are between -1 and +1diopters!
This means that without glasses, you can see quite well (sometimes better than the glasses that you are wearing now, because a strong glasses have a disturbing reduced or enlarged effect).
Reading glasses are however still needed (see below).
Therefore it was also requires that you have come to a "glasses-age", before we could consider this operation.
However, it should be stated that the available artificial lenses increased depth of focus, then your own lens.
So you can sometimes read titles in the newspaper without glasses, especially if we try a myopic view of what to do (about -1)!
So with that one eye can reasonably read without glasses and with the other eye,you can look far without glasses!
To really see perfect close and far, a slight correction with glasses remains necessary.
3 weeks after the operation, we will therefore always propose  a double-vision glasses, which contributes to your own sense: especially in visually demanding tasks: driving / watching TV and reading.

BENEFITS OF A CATARACT EXTRACTION COMPARED TO LASER TREATMENT:

- It has years of experience in this operation in a large number of people: they know that the available IOL is well tolerated for many years. Today, the doctors dare apply an artificial lens even in young children with cataract (may occur in certain diseases or accidents)!

- A faster recovery and stabilization: the day after surgery, there can be seen for far.  Light reading glasses (+3) is required to read! Sometimes there can be a tense on an attachment, it still disturbs vision without glasses! Which can easily be removed after a few weeks. After 3 weeks, the glasses gets adjusted. Usually the glasses remains stable for many years. Probably 1 or 2 years after surgery, a little vision impairment can occur, caused by blurring of the lens capsule. This should be retouched once with laser in the cabinet (not in the operating room): painless / 2 minutes work! After that the sight is really stable.

- Stabilization of the glasses: the glasses changes no further, because the lens can not evolve anymore(= thickened or obscure).
In normal people with an intact lens, the glasses should be adjusted every 2 to 3 years, especially from 45 years, to improve reading - the surgery is largely paid back and takes personal + € -460 per eye (more if you want a private room, less if you have a hospitalization insurance). Laser treatment is not refundable and will cost € 1500 per eye.

- It's no extra surgery: high refractive disorders gets faster cataract (= age-related lens opacity) than others. These persons will need this surgery a little bit earlier than others. So you can take advantage out of it. Your glasses won't be that strong anymore after surgery and you will not go through reduction of vision during the formation of the cataract!

- For all cataract patients with operated eyed, the eye pressure is lower than before, what constitutes a protection against high eye pressure (glaucoma). Especially for high far-sighted people is that an advantage, because these patients are predisposed to sudden pressure increases. Due to the operation it could not happen.

- If the calculation of the lens strength isn't what you expected  (= more than +1 or -1), an additional lens can be applied so that the result is adaptable. Remaining myopia can also be adjusted with a laser surgery. Knows well that a residual myopia in 1 eye is ideal for reading without glasses! We therefore believe that a -0.50 to -1.00 is ideal after surgery!

- If you want, you can wear for this slight correction contact lenses, which is after laser treatment hardly possible (by the change of the cornea)

- A lens in the eye is optically more clear than a lens grounded on the cornea. With laser: less chance of loss of contrast and loss of best corrected vision with optimal correction. 

DISADVANTAGES OF A CATARACT EXTRACTION COMPARED TO LASER TREATMENT:

- A half-day hospitalization is needed. Laser treatment is without hospitalization.

- Afterwards, the glasses needed to be worn for reading, because the IOL hasn't reading capabilities, such as the natural lens!
However, we regularly try to make one eye slightly nearsighted (-1.00  to -1.50 diopters) so that one eye can read without glasses, while the other eye can see well without glasses for far, that is called "monovision".
The difference between both eyes was very well tolerated, although it sometimes requires several weeks adjusting. For fine reading material, you still need reading glasses. For optimal distance vision (TV watching / driving at night), some patients opt for a distance glasses.
Alternatively you can opt for double vision glasses. The glasses are much lighter than before and need to be worn less than half the time!

- There is now the possibility to implant double-vision lenses. There are four problems:

• distance vision is not as pure as with simple lens implants
• spacing (= computer work / housework / work on bench) falls out
• Near vision is rather close (= 30 cm) and not as pure as with simple reading glasses
• night vision brings problems: jet figures around lights / glare

Some people are happy with double vision lens implants. 10% of people are so dissatisfied that they make us decide not to opt for double-vision lens implants!

- In degenerative areas (= wear spots) in the retina (especially with myopes) there can eventually occur problems (eg cracks). Therefore, your eyes will always be checked pre-operatively for wear spots. Possible degenerations can be secured with laser. However if a retinal detachment would happen (very rare), it can be surgically repaired in a university hospital.

- For every operation in the eye, an infection can occur. It should be set clear that the chance for inflammation is very unlikely in cataract surgery.
It is very important that any infection is detected early. Therefore you will have a control in 2 days and 5 days after surgery.
An infection can usually be solved with a local and systemic antibiotics. Rare is a new surgery (vitrectomy) necessary, in order to wash away the infection. Extremely rare is a lost track. Usually, it occurs by older people with weakened health and diabetes.

- Color Shift: our own lens is greeny. The artificial lens is perfectly clear. This makes the colors brighter to be seen, but also a little different. We speak of a "blue shift": red becomes a little fuchsia - gray may turn in to purple.

- Hypersensitivity to light. Usually this is temporary and this is caused by a mild inflammation. Sometimes this can be permanent, because the new artificial lens is much brighter. Some patients suffers from these new clarity and can't adjust to it. Faster wearing sunglasses is usually a solution.

HOW GOES THE OPERATION: 

Under-drop anesthesia, your own lenses got "squished"  out off your eye through a small incision (3.2 mm). Afterwards a clear artificial lens get applied in your eye and there has to be made 1 to 3 stiches. The operation, which is about 1 / 2 h duration is well tolerated. An eye is a very strong body! After the surgery, a strong bandage is applied that after half a day can be removed.
Then the patient can see with the operated eye. Contrary to expectations, the patient experiences little pain after surgery.
The patient may go home a few hours after surgery. It should now have two weeks that a shell needs to be worn at night to protect the wound. Otherwise the patient may do everything after surgery (= bending / lifting objects / watching TV and reading).
  
After 1 week, the other eye gets surgery. In the week between the two surgeries the patient expierence  "difference in the strength" between the eyes. The second eye is corrected with a temporary contact lens, which can be worn day and night.
For instance, the distance vision without glasses is quite well after the first surgery. 
After the first operation you will also be able to read. You can borrow reading glasses from us to get trough the first weeks.
The final spectacle correction is approximately 3 weeks after the last intervention required.
Usually they are a double vision glasses (see above), with a very slight distance correction and a clear reading correction (like all people of your age!). You do not always have to wear glasses, but to their own requirements for visually demanding tasks such as (TV - auto-drive). Only then can the final outcome be assessed. In 100% of the cases the strenght of the glasses is more important after surgery. Almost always, the view is the same as before the operation or even better (because the implanted lens, don't reduces or enlarges the images like glasses do).
Rarely is the visible result not what you wanted because of other eye diseases, which previously were not shown.
Sometimes problems can occur during surgery, which limits the result, although this is rare (eg infections / unforeseen difficulties in finalizing the IOL).
